Ciguatoxins exert their effects primarily by binding to voltage-gated sodium channels (VGSCs) on nerve and muscle cell membranes. They cause a persistent activation of these channels by shifting the voltage dependence of activation to more negative potentials, leading to prolonged sodium influx, spontaneous depolarization, and repetitive neuronal firing. This hyperexcitability explains the characteristic neurological symptoms of ciguatera. Additionally, ciguatoxins may inhibit certain potassium channels and affect calcium channels, contributing to the wide range of clinical effects.
